Or fill it in below and print it. Nothing you type on this page is saved or sent anywhere, so print or choose Save as PDF before you close the page.

How to use it

  1. Copy details from your vet’s records, certificates and medicine labels. Write them exactly as they appear.
  2. Add a line after every vet visit or treatment, including the next date your vet gives you.
  3. Print one per dog. Keep it with your dog’s papers and take it to new vets, groomers, sitters and kennels.

This is your own copy, not a veterinary certificate. Kennels, groomers and day cares usually want to see the record your vet issued, so keep the originals too and note where they are.

Questions people ask

What goes on a dog vaccination record?

The date given, the vaccine exactly as your vet wrote it, the clinic or vet, the batch or lot number if shown, and the next date your vet gave. The table above has a column for each, plus where the original certificate is kept.

Is a shot record the same as a vaccination record?

Yes. “Shot record”, “vaccination record” and “vaccine card” all mean the list of vaccinations your dog has had.

Will a kennel, groomer or daycare accept this as proof?

Usually not on its own. They want the record your vet issued. In England, licensed kennels must see an up-to-date veterinary vaccination record, and many US facilities ask for written proof from a licensed vet. See what kennels ask for.

I’ve lost my dog’s vaccination records. What now?

Contact the clinic that vaccinated your dog and ask for a copy of the vaccination history.

Do I need a separate record for each dog?

Yes. Print one record per dog.

Does this tell me which vaccines my dog needs?

No. Your vet decides which vaccinations and treatments your dog needs and when. This sheet only records what was done.

Can I keep it on my phone?

You can save the filled-in PDF or photograph the sheet. Paws & Days keeps the same history on iPhone, with optional reminders for the dates your vet gives you.
